Who’s eligible for Silver Chain Home Hospital care?

Before you refer a patient to Silver Chain Home Hospital, find out who’s eligible to receive our acute and sub-acute in-home care.

 Are your patients eligible?

Home Hospital care services are available to people living in their own home, or in a residential care facility.

Patients must be:

  • Referred from a public hospital/ED, GP, specialist or aged care facility
  • In need of short-term acute and subacute care that can be delivered safely in the home
  • Medically and mentally stable
  • Medicare eligible
  • Aged 13 or over and able to provide consent
  • Able to communicate effectively, either directly or through an interpreter

Patients must not:

  • Require an emergency response
  • Be more than 22 weeks pregnant
